                                 Patient's Guidelines

The following guidelines were developed in the interest of setting a clear
understanding of what is expected from both doctor and patients in our practice.
We regret any inconvenience this may pose. We look forward to hearing any
suggestions you may have to improve our service and your health.

As a patient, you have to arrive for your appointment promptly. Dr. Sun's on-time performance
is 95%. Appointment time of 9:00AM means just that time, not 8:30AM or 9:30AM. If you want
to cancel your appointment and reschedule, please do so in advance (prefer al least 24 hours
prior).

All new patients are expected to bring along as many medical records as possible, including
previous laboratory tests, X-ray/IVP, renal scan, kidney sonograms, cat scans, MRI/MRA,
angiogram, kidney biopsy, etc.

All patients should also bring along all medication bottles and a list of nonprescription drugs
with them in new or follow up visits.

You need to bring in referral from your primary care physician if you have HMO health
insurance, and also, you need to bring along your insurance cards with you if you are a new
patient or if you change your health insurance plan.

Lab tests for follow up visits should be obtained 1 to 2 weeks prior the visit. You will receive a
computer generated requisition slip for a Quest Diagnostics lab. You can have your blood test
done in our office, or you can call (800) 631-1390 or visit www.questdiagnostics.com to locate
your nearest facility.

Interpretation and discussion of lab test/imagin study results should be conducted in person
visit and you can make an appointment for that. The staff are prohibited from releasing results
over the phone or fax without doctor's review and authorization. However, critical lab and
imaging results will be reviewed urgently over the phone.

Prescription renewals should be obtained during your visit. Do not rely on the capacity of the
doctor to simply phone it into the pharmacy.

Dr. Sun and his associates cover weekends on a rotational basis. Covering doctors do not
have ready access to your medical record. It will be difficult to handle your complex medical
matters. So, if you have medical emergency, please visit Beth Israel Medical Center at
Manhattan's emergency room and mention that your doctor is Dr. Wei Sun.
